OMHA no longer have paper copies of NRP forms available. All NRP form request are completed online now by our Registrar.
After April 24th, parents will be able to complete an NRP form request through the BDMHA website that will go directly to our Registrar.

ALL INFORMATION MUST BE PRESENT AND ACCURATE BEFORE A REQUEST CAN BE SENT TO OUR Registrar.

Player's DOB
Player/Parent Email Address
Name of Home Association (ie. Burford Minor Hockey)
Home Association Base Category (ie. C)
Age Division (ie. U14)
Name of Receiving Association (ie. Norfolk)
Receiving Association's Email Address (You will need to find this out prior to submitting form***)
Category Player is trying out for (ie. AA or A)
