This form is for Team Managers or Coaches to Apply for Tournament Travel Permits and Approval of Rosters, and requesting a cheque. This form can also be used when teams travel for exhibition games outside of OMHA Events such as out of Province or Country Tournaments. If fields are not applicable please fill with n/a.

Burford Hockey teams that are looking to register for tournaments and require early payment please fill the form out. Once you have collected funds from the team and roster is approved payment back to BDMHA is required. Burford Minor Hockey Association will require the team to submit the tournament fees 30 days prior to the tournament date. Please note. Partial Payments will not be accepted. It is the responsibility of the coach or parent rep to collect the entire tournament fee 30 days prior to the tournament taking place. Team travel permits will not be issued until full repayment is made.
